When I first saw it, I tried to reproduce the bug, but couldn't :(

This was when I was working three jobs and basically nnon-stop from
7am to 7pm, so I didn't have time to look at it further.

It may be caused because he's using upstream_servers

I'll try to find time in a day or two to look at it, before my trip.
I may have to test it with the Windows version of MaraDNS from a cyber
cafe.

I think fixing bugs is far more important than features; a DNS server
should be simple and stable.
