thanks for the info.
f1040--2008.pdf has the same issues with evince, probably that's related to
the way it was created.
but the newer versions 2009-2013 works pretty fine.

Replying as to how I see this apply to Okular.
In Okular one needs to enable "show forms" before US IRS forms can be filled
in and checkboxes clicked. Usually there's a message at the top of the window
giving the user to enable this option, and it can also be done via
View -> Show Forms
