I have also been having a problem with PulseAudio not starting after a recent upgrade.

I tried cleaning up the ~/.pulse folder but it did not help.

However, default.pa was causing PulseAudio to attempt to load module-cork-music-on-phone, which no longer exists.

Changing the line in $HOME/.pulse/default.pa from
load-module module-cork-music-on-phone
to
load-module module-role-cork

solved the problem.
