MHD_create_response_from_fd_at_offset64() and the deprecation of MHD_create_response_from_fd_at_offset() creates dependency hell which is unnecessary. Can't you just test for sizeof(off_t) and sizeof(size_t) and only issue a warning if they are different? I'm sure there are some docs on glibc LFS support, and how to do this better.
