The URLs I have given to the bioinformatics paper are (mostly) NCBI PubMed URLs.
As a start I suggest to render it identically if the PMID is presented but no 
DOI.
If there is both, then some &nbsp;<small>(<a href="...">PubMed</a>)</small> 
behind
the title would be fine.
