I'm also having this issue on 12.04 and 14.04. For me, adding the manual pin solves the problem after power off/on but does NOT solve the problem after channel switch, where a manual reconnect is required. Also, on both 12.04 and 14.04, I'm having occasional issues where I'm getting a keypres
