This bug seems to persist in the current evince (2.22.1.1-0ubuntu1) -
this is quite a serious bug!

Binary package hint: evince
If I check the yes box in a form using acrobat Reader, then save it, and
reopen it in evince, Evince shows the No box as checked.
To reproduce:
1) Download form from www.irs.gov/pub/irs-pdf/f1040nr.pdf
2) Using acroread, check Some Yes boxes on

If you try to change the width of a column in the list of song, the application
is not behaving the way it shoud: moving the cursor to the right moves the
column to the left...
To reproduce this bug, just try to change the width of a column
