I too had the same problem in intrepid. Thanks to persia in ubuntu-motu
channel, i was able to make it appear.
I had two entries for epiphany in menu editor one was checked and other
not.

from a different machine (like ldap, nis etc)

unless you are locking and unlocking as root. Please try it again

Is your password expired ? Can you try changing your password and see
whether you are able to unlock with the new password ?
If you still see the problem can you post "/var/log/auth.log" after
trying to unlock the screen.
