This is hard to replicate since it seems to occur on a -very- specific
occasion, almost randomly. I didn't run into it much.

It occurred in the first nautilus window opened after boot.

I should mention I did have Visual Effects on, but after closing and
reopening the window, having VE on made no difference (no problem
occurred).

I have experienced this too.
Furthermore, the problem window exhibited another bug. After failing to
rename the new folders, I moved up a few directories and attempted to
make a new folder in Home. The new folder appeared in the same location
as the earlier problem (2 levels deeper in directory t

Yes, I have them enabled. I don't see how it related them at all,
however...
The problem is very hard to reproduce, I couldn't get it to happen again.
I'll let you know when it does happen.
