The about dialog is a code bug fixed upstream as mentioned, the other
problems sound like a bug in your theme and you should report it there

i edited /usr/share/themes/Greybird-dark/xfwm4/themerc
added this line:
show_app_icon=true
You can see the issue in the screenshot in the top left, this affected
every theme that shows the icon for the menu
This also affects notifications (song change/hit play)

Here are the icons i pulled from my backup
This can be fixed on a existing install by doing this
sudo cp -a ./hicolor /usr/share/icons/
sudo touch /usr/share/icons/hicolor
sudo gtk-update-icon-cache
