I attach here a tar ball containing 2 files. In Mines1.png, the tile
under the mouse cursor showed no flag before screenshot, even though I
had right-clicked on it. Mines2.png shows all mines in the same game.

Yes, it's reproducible, if you play the game long enough.
Open the Mines application and choose a big grid. Play the game (no error
please ;-), marking known positions of mines with a flag, by right clicking on
the corresponding tiles. After ten moves, or so, flags will begin to be drawn
