So, I tried the same sequence for the "Run terminal" action, and I got
the same problem. It means, I set "Ctrl+Alt+t" shortcut to the "Run
terminal" action, after that I reset it (now it's in the "Disabled"
state), but I still can run the terminal by pressing the "Ctrl+Alt+t"
shortcut.

thank you for your bug report, do you use the desktop effects option? is
that specific to some shortcuts or do you get the same bug to take a
screenshot for example or go to the next track?

One more comment here.
Even if I use this hotkey (in this example "Ctrl+Shift+!") for an other
action, it still works as it was applied before (in this example for the
"Move window to workspace 1"), and doesn't work for the new action at
all.
