Had exactly the same issue tryingto get Xvnc to work reliably.
Swicthing localhost for 127.0.0.1 in /etc/xinetd.d/vnc fixed it.

I had the same error (gdm-simple-slave[3514]: WARNING: Unable to connect
to display ::1:1) but found it was a problem with the parameter I sent
to the VNC server. I was using vnc4server and used the parameter
'-query localhost' when starting up Xvnc. In fact, I needed to use
'-query 127.0.0.1'.
