It can happen under various circumstances:
- a bug in libxklavier library
- a bug in X server (xkbcomp, xmodmap utilities)
- X server with incompatible libxkbfile implementation

Update: The above build is running virtualized in VMware. There was a
kernel update for Lucid. After reinstalling VMware tools the keyboard
now works with gdm. It is not clear why the keyboard failed to work with
gdm but worked in the main user interface.
