Public bug reported:

After upgrading to 11.10, Nautilus had no icons, just generic sheets of
paper icons (see image).

I was upgrading from a system that used the Radiance theme. To repair
this problem, I had to reselect the radiance theme in System
Settings>Appearance, and reboot. (see result image)
