A fix for this has been merged upstream:
https://gitlab.freedesktop.org/pulseaudio/pulseaudio/merge_requests/134
I believe this should make it into the 14.0 release.

Not an upstream bug after all. I downloaded the latest release build,
unpacked it in a clean VM and observed that the icon for DDG in the new
tab page is reasonably high resolution, unlike the one in the Ubuntu
builds.

Thanks for the upstream bug Carlos.
I don't think it's a bug in the GNOME stack, given that you observe it only in
chrome/chromium and electron apps.

It sounds like this might be a symptom of bug #1616650 (although the
claim that history isn't affected would go against this hypothesis).
Could you try enabling the experimental refresh-app-awareness flag,
restart chromium, use it for a few days and comment back here whether
the problem persists?
so this apparently needs re-testing with appropriate triggers, or maybe
tightened dependencies on some packages.

Thanks for the bug reports. Sorry you're having trouble getting the
right attachments, I can't see why...
My first guess here is that maybe you're using Nvidia and the nvidia-390
driver? If so then please try booting an older kernel like 5.0 and see
bug 1851162.
