Have the same issue with Ubuntu 17.10
$ uname -a

"aplay -l" gives the following

After upgrading from 17.04 to 17.10 I also had this issue with error:
> pulseaudio[6267]: Module "module-switch-on-connect" should be loaded
once at most. Refusing to load.
Fixed like in #4 by commenting load-module module-switch-on-connect out of
/etc/pulse/default.pa
Have not tried any plug-
Upgraded to Kubuntu 15.04 and with that came nvidia 346.59
( 346.59-0ubuntu1 )
$ uname -a

I restored back to opensource drivers and on login it asked me to report
a bug with fglrx. I did so and it created this bug #1259709

I think the duplicate status is wrong.
I think this is NOT a duplicate of bug #1068404 because **this** bug has
the error:
[ 593.127] (EE) Failed to load /usr/lib/x86_64-linux-gnu/xorg/extra-
modules/ext
I can confirm that this fixed the issue for me. Seems like the
application is faster too. Nice work.
Now running: simple-scan 3.4.3-0ubuntu1.
Still running Ubuntu 12.04 LTS.
I do not see where I can change the tag from from verification-needed to
verification-done. Perhaps I do not have rights
I think I found how to change the tag from from verification-needed to
verification-done and did so. If I did it in error - my apologies in
advance.

can't scan more after "scan all from docum
After some "apt-get upgrade" to Ubuntu 12.04-LTS I now get this exact
same problem with a Brother MFC-8660DN.
I don't know which upgrade caused the problem or when it started, but
with the original Ubuntu 12.04-LTS I was able to make multiple runs.
