With current ubuntu LO (4.3.3) the issue stands.
As I wrote upstream, the added text seems to be the control's 'label'
property.

Those upstream patches seem to have been late for utopic... At least, I
still don't get 2.1 on utopic - whereas trusty with your ppa was fine.
Any chance you would update your ppa for utopic one last time?
Thanks!

whole network connection does not work.
Does not seem to be a mere duplicate bug.

I'm NOT EXPORTING, but PRINTING!
I don't see any PDF options for printing. Other than the printer
language selection (PDF/PS), of course.

However, exporting to PDF - irrespective of export options - produces
the same result here it cannot be a 32bit/64bit problem, can it?

I tried 3.4.4 from above-mentioned ppa - same result.
However:
If an actual PDF FORM is created, the overlays disappear. The form elements are
rendered differently, however.

Public bug reported:
Printing a libreoffice document containing form elements (checkboxes
etc.) get superimposed with additional text: e.g. 'Checkbox' besides a
checkbox etc.
Switching 'Printer Language Type' to PostScript solves that - that
setting is, however, not saved and has to be manually
