Add these lines to /etc/rc.local (before exit 0)
cp -R -p /dev/video0 /dev/video2
cp -R -p /dev/video1 /dev/video0
cp -R -p /dev/video2 /dev/video1
rm -rf /dev/video
This swaps the video devices on startup.

normal solves the problem, Unfortunately I'm not sure what caused it in
the first place..

I still have this problem.
I can't maximize windows on the second monitor IF AND ONLY IF the
window is bigger than the monitor size, this is often the case because
my primary monitor is bigger than the secondary, dragging windows from
smaller to larger works as expected but not the other way
This bug still exists in 12.04 Evince 3.4.0 , like #4 suggested it is
only certain files, the file I have problems with was created by TeX
with beamer plugin.
I will attach it but only if needed.

precise Lubuntu up to date on old dell inspiron has this issue. Seems to
be related to power management, wifi will associate on battery -- but
will have no connection, pinging router results in 100% packet loss.
Not sure if this is general..

I've tried to reproduce this with my Vaio laptop. The Inspiron that had
problems used a Broadcom wifi card, as does another laptop that can
reproduce this.
This seems to be simply a problem with the broadcom wifi drivers in this
case.
