Title:
  Gnome-Screensaver unlock: Dialog for password does not appear [Intel
  GMA 4500]

Status in “gnome-screensaver” package in Ubuntu:
  Confirmed

Bug description:
  Occasionally the dialogue to type in the password does not appear. I
  can only see the wallpaper.

  Workarounds:

  1. switching between ttys back and forth until dialogue appears. I
  noticed, loging in to one of the ttys increases the change that the
  dialogue will appear when jumping back to tty7.

  2. switch to a tty, login, and 'killall gnome-screensaver'

  
  Sometimes - if I don't log out - it works perfectly for several days and 
dozens of locks/unlocks in a row. I do not have this issue on any other 
graphics card (e.g. Intel SandyBridge). How can I track down what is causing 
this issue? 

  
  System: Precise 64bit 

  lspci: VGA compatible controller: Intel Corporation 4 Series Chipset
  Integrated Graphics Controller (rev 03)

  driver=i915
