I attempted to recreate this bug in Ubuntu 12.04 LTS
I created a txt file named Test%. I as able to open the file as well as use the
command ls -l on the file as the OP did. I could not recreate the issue posted.
I also tried naming the file art%253A10.1007%252FBF01034471.pdf as the OP had
done.
Attempted to recreate the bug by creating a pdf using libreoffice
(File->Export to PDF) with a similar naming convention utilizing %25 but
did not experience the same bug.

I created a directory called evincetest on my desktop.
I created a document called test%ubuntu
I made a copy of that document and renamed it test%ubuntu.pdf
I exported a libra office document as a PDF to the same folder called
fish%ing.pdf
evince can see all three. I can open the P

This did not/does not occur on Quantal with the 3.6.x kernel, so I'm
pretty sure this is a Raring specific/3.8.x kernel series issue. On
Quantal, my Wifi card was only supported by b43, so I've only begun
using bcmwl with Raring.
I think bcmwl added support for my Wifi card when the 6.20 firmware

How to reproduce a bug:
- start cheese (or skype)
- the screen is black
- wait a minute or two until you see your webcam light is on
- close cheese
- start again cheese
- now you can see a picture from a webcam
Expected:
- video should be shown after you start cheese

I'm running 13.04 with all packages up-to-date including proposed. Also
had exact same problem in up-to-date 12.04 and 12.10.
I don't know how to do an apport on xorg.

I've been suffering from this issue almost at every shutdown since I
installed Ubuntu (first 12.04 then 12.10), it disappeared when I
switched to KDE a couple of weeks ago. The more the system is used, the
more likely is the issue to happen, but I was never been able to
reproduce it systematically.
