Here is how I reproduced it:
I created an iSCSI volume on my SAN that is only 2 GB.
I created a XenServer SR based on this SAN volume.
I created Primary Storage in CloudStack based on this XenServer SR.
I created a Disk Off
I did look at it, but haven't had a chance to try to repo.
I should be able to try to repo it today.
