Upgrade CommonJS modules support to Modules/1.1.1

Our current commonjs modules support is based version 1.0 of the Modules
specification. Modules/1.1 had some points that made it difficult and/or
impossible for us to implement to spec so I worked with the commonjs guys on an
upgraded revion, 1.1.1, which we are able to implement.
