Luke W's Form Design book says to use smart defaults and that a good
solution would be to ask people to explicitly select the month, day
and year they were born. Also let them know why you are asking for
this information.

Suggest looking for someone who does custom note printing in your
local area. They often will sell them. Outside that, the usual places
that 3M (or other knock offs) sell Post-Its. These include Amazon.com,
Office Depot, Staples, WalMart, etc.

I echo Erik and Mehdi's suggestions. Rest assured you are not the
only person in this position. I recently wrote a couple blog posts
about just this problem after seeing others in a similar situation.

I suggest a read of Luke Wrobeleski's book Web Form Design book in
terms form design best practices. In fact, anything speaking to
eCommerce best practices would also be relevant given that in the end
it is all about converting the visitor.
From a best practices stand-point Forrester publishes a
I love Fireworks for wireframing, but it does have some limitations:
- single Master Page can be created only
- a lack of stencil options
- no macros
Some pros:
- ability to export files quickly to PDF, HTML, JPEGS, etc and create
quick functional prototypes
- pixel based sizing means you are

By stencil limitations I meant the lack of pre-made stencils
available. OmniGraffle has GraffleTopia. There is little similar for
Fireworks. Not that it is hard to create your own.

You can crawl the site using a client that creates sitemap XML files
(the kinds used for SEO), then use OmniGraffle and a bit of
Applescript to turn that XML into a graphical sitemap. Not sure how
it will behave with super large sites, but might be worth a try.
Details on how are in this thread on
Stephen Few's other book -- Show Me the Numbers is also worth a look
as it has some great dashboard examples and speaks a lot to effective
layout and information design.
