I had the same experience earlier and then reinstall fixed it.
While we investigate, is it possible to get a dump of actual rules from the ssh
or something and a quick way to delete all of them, even if "purge all" because
the config.xml reload should create fresh again right?

Fresh install of the latest pfsense, I initially set up some NAT port
forwarding, removed them, and tried to re-add them. Unfortunately the
2nd time around none of my NAT forwarding works.
I have even tried rebooting the machine as well, and the firewall rules
