I' m the developer of the herold HTML to DocBook converter and I would
like to implement the hyperlink to footnote conversion, because it
sounds like an interesting feature to me. Do you have an idea about what
the generated DocBook should look like?

If you are using DocBook 5, then element names in any stylesheet
customization need have the DocBook namespace prefix (d:entry) as described
here.

I have had very good luck with Herold (http://www.michael-a-fuchs.de).
I'm usually not fortunate enough to have strict xhtml, so we do some
pre-processing (usually on well-behaved, but idiosyncratic, html), tidy it up
into xhtml, then run Herold.
