The XSL 1.0 standard did not have elements for PDF bookmarks, but the
XSL 1.1 (dating from 2006) standard does. Now it seems all XSL-FO
processors support the standard fo:bookmark-tree and fo:bookmark
elements. That means the custom bookmark elements that had to be
