Actually the base with the preamp will also drive a hi-Z input just
fine. Whether you bypass the preamp or not is a matter of choice.

You didn't say if it was a D-104 with or without a preamp in the base of
the stand. The D-104 w/o preamp is a high impedance mic and therefore
doesn't need a transformer. The D-104 with a preamp in the stand has a low
impedance output but the preamp should be bypassed since it isn't needed
in this

You don't need a transformer
Wire color codes vary, but _usually_ microphones are wired so that
the shield goes to the "sleeve" of the connector, white goes to the
"tip" (PTT switch), and red goes to the "ring" (microphone).
