https://bugs.launchpad.net/ubuntu/+source/unity/+bug/1292217 may or may not be related; but simply allowing the screen to timeout lock is not sufficient for me to reproduce, nor is plugging/unplugging an external display. Suspend/resume seems to be the trigger. -- You received this bug
