The question is, is there a common component failing? and it looks like
that it is the LDMOS from the posts I am reading.

I agree with Rick just keep the drive power to the minimum needed to get
the 500 watts out. Over driving will do nothing but distort the signal and
most likely eventually damage the amp. I use 18 to 20 watts to get 500 out
on 160 mtrs and a bit more to get same results on the higher bands.

Try starting with a lower drive of 12 to 15 watts for the first pass with
the auto tuner. Oh and be sure the RF CONNECTORS ARE ALL GOOD AND TIGHT..
A POOR CONNECTION OR A BAD CONNECTOR WILL CAUSE A FAULT WHEN HIGH WATTAGE
IS APPLIED.

I got mine already loaded it, .all is good.. .. The amp still works great
28 watts of drive from my K3s and 1500 peak out of the KPA 1500 on 160
meters
