I think I have figured out how you made the velocity estimate work in 
the HAL encoder component for all of the different modes.  One thing is 
this scheme needs to have the "timebase" counter available.  The 
timestamp scheme I put into my PWM controller FPGA only brings out the 
timestamp from the last encoder count, not the timebase.  I'm not sure 
there is a really good way of making this scheme work without also 
getting the timebase read out by the driver.  Any comments?

If that is necessary, I need to add two more bytes to the readout to 
transmit the timebase count.

I still haven't worked out how it deals with overflow of the timebase.
