Although not a requirement, I have used corporate logos on my DoC's. They were not on letterhead but nonetheless the Logo/Trademark was on the DoC. I viewed it similarly to a CB Scheme Safety report that requires the Trademark and a copy of the marking plate to further assist in identifying the
