I removed all mouse bindings for 4/5/6/7 and all buttons show with xev.

it does!

Do you mean that the extra buttons don't work in, say, Firefox, or that
using the wheel dosn't switch desktops but still scrolls the page in
Firefox? If the latter, then do `enlightenment_remote
-binding-mouse-list` to see what mouse buttons are currently registered

never had any problems with it.
maybe it's the Protocol you use.
"ImPS/2" works for me just fine.
