We are in the process of re-applying penalty thresholds after suspending
them due to COVID, and I thought this thread would be a good opportunity to
bring attention to a few related bugs we've come across:

Thanks everyone for your responses. I removed the branch level thresholds and
placed the system level threshold again. Looks like the bug mentioned is the
issue at play. We can see the issues from the branch when logged in at the
