John,
It's true that the auditor table tracks the user and workstation when
patron records are edited, but no entry is added to
auditor.actor_usr_history when the record is first created. So creator info
is not tracked, only edit info (which is sometimes, but not always,
helpful).

We created a local solution to this circa 2012. We store a creator and
create_date in actor.card, so we can see who issued each library card.
It doesn't capture every edit, but does show who created the account
initially and who issued any replacement library card(s).
I've added details to
Hi Deanna,
The fact that Evergreen doesn't currently capture the staff user and
workstation when a patron record is entered is missing functionality. There
is an existing Launchpad bug for this:

Hope this helps,

Michele M. Morgan,
We use Envisionware's staff register and e-commerce solutions. It cost a chunk
of money, but it works pretty well. The support has been excellent, although
we had some installation issues that I blame mostly on the pandemic (we
installed in Aug. 2020).

I find it perplexing that Evergreen does not indicate which staff member input
a new patron record. I can see which staff member checked an item in or out,
so why can I not see who set the account up? It is incredibly useful data
because it can help me spot if a staff member is struggling and
We just put it as a note that says "registration" and then archive it. And then
initial the application :)

I would like to figure out where to include staff initials on our new
patron accounts. Does anyone require this info? It is really helpful when
training new staff to know "who" entered the data.

Good Morning Terri,
I've observed this behavior in the past (it's been a while), and I know it
can be an intermittent issue. There are a few known bugs related to this
issue (and depending upon which portion of the interface you are
