OutofOff not working 4 people outside

We have: sendmail recieving mails for our domain. Sendmail forwards all the mail to our 2 exchangeservers.(5.5 sp3) When an employee configures the out of office, it works for internal people but not for people from the internet. Yes we have a firewall, but it doesn't block smtp, Who
