I have the barracuda 300 appliance and love
CanIT Pro or Ironport, both appliance based. Ironport doesn't have
greylisting, but doesn't need it eitherpretty much no spam comes
through.
Both have high quality support.

I know Barracuda does all but #5. They have been known to give free
appliances to test with and they're very inexpensive
All but #1 you mean?

If OSS is an option, try Maia Mailguard. I run it over
FreeBSD/Postfix, and it works extremely well.

I use a product called SpamTitan, which has all those features exept

Very configurable.

Take a look at IronPort as well. I've been happy with their appliance.

Tumbleweed's mailgate will do all that except greylisting but it has throttling
for misbehaved IPs. .

MailMarshal. Does everything you want and more. It's easy to use and to
configure(tonnes of default rules/config already setup for you). And it just
works. Can't recommend it highly enough.
