Have you tried rebooting? Have you tried reloading the MIDI drivers
through the MIDI menu? Or switching from VST to MIDI and back? (Don't
ask me how I know!)

Yes.

I have rescanned the midi devices and done what I know how to do with
that.
Have you tried it on multiple files? An old one? A new default
document? What about one of the other default files (jazz font?) in
the components folder?
