I am also having this issue (MX2004 7.2). My guess is that it comes down to whether you use the Extension Manager or not. I build them in the WindowSWF folder hardly ever use the Extension Manager for my own panels. If there are only a couple panels installed they seem to work fine, but when I have more than 3 or 4, I get the same quasi-randomness that you described. However when co-workers install the panels using the Extension Manager, they never have that problem.
