Thank you very much, I corrected the errors ;-)
And now I got tit also with the table: I had to put the table into an own
block and then setting this block to span="all".

1/ fo:external-graphic is not allowed as a direct child of fo:flow (see
following line)
2/ your fo:leader is longer than available column width
You shoud use leader-length="100%" instead
3/ the sum of fo:column width is grea
