The plugins should work with FrameMaker 10 but have not been tested with it.

Experimenting, I set the Note tag to exactly match the paragraph tag
to exactly the same settings on all tabs of the Paragraph Designer
except for Numbering. They still don't behave the same.

Aha. The extra lines were coming from three-point empty paragraphs.
Nothing to do with formatting except that somebody who worked on this
doc spent way too much time on it.

This template that I inherited uses a code character tag to make
commands in headings appear in monofont (Courier New).
The hypertext links on the TOC entries end at the first font change.

We ran into the same problem when one of our writers accidentally was given a
64-bit Win7 machine, whereas the rest of the department had 32-bit machines and
no problems.
We solved the problem by replacing his machine with a 32-bit one like the
rest of us. Doesn't really solve your problem,
