We're using unstructured FrameMaker, and we've simply imported small
snippets of text into larger Frame files.

What type of text insets are you using? I have seen this behavior with
XML text insets created when opening an XML document. XML text insets
created by importing XML fragments into existing FM documents behave as
expected.

When I update a book file, I select the "Update All Text Insets" check box.
