This is old-school, but If your line spacing is standard, one option would
be to place a new flow on the master page(s) just wide enough for the largest
line number.
On the body page(s), you would insert into that flow a series of autonumbered
paragraphs with no other content. (They should have
until the next paragraph
style. The feature itself is nice, but the implementation is..."not brilliant".

What's the limitation of autonumbers that would make it impractical?
I've done line numbers in code listings that way.

It doesn't. Ed's proposal numbers paragraphs, not lines.
