In the FM PDf workflow you should be able to set bookmarks to init caps
by using TimeSavers. Worth it's weight in gold that one is.

As far as Word goes, well, there isn't much to say other than good luck. In

In FrameMaker, you can type your heading using initial caps, but set the
paragraph format to use Uppercase (Paragraph Designer Font tab, check
Uppercase). When you convert to PDF, the bookmark will display how you typed
it (initial caps), while the heading itself will show in all
