Running a Pismo 400 with 576MB RAM, 60GB HD, OS 9.2.2 w/many third party utilities and hacks.
Problem #1 really sounds like RAM or loose processor.
Problem #2: Apple Audio Extension. Had the same problem. Would have to reboot with extensions off and then switch back to get going again. Intermittent, generally cropped up after a crash while running, even with extra system heap. Given the soup of utilities and add ons in my system, I disabled it. Don't miss it. No more crashes during boot. Not sure if it is necessary on the Pismo (if wrong, please enlighten me - conflict catcher reports it as being an update for audio software for PCI macs).
