$ gcc -E -Wp,-P -xc - </dev/null

No output, correct.

$ gcc -g3 -E -Wp,-P -xc - </dev/null

Lots of cpp defines in output, incorrect.

$ clang -g3 -E -Wp,-P -xc - </dev/null

No output, correct.

AFAIK no "#define"s should be outputted by the
cpp pass. Quite the opposite, cpp should expand
them all. But with -g3 for some reason this doesn't
happen.
