For PR46358, I added the keyword patch, the patch URL, and a comment, and
then proceded to add en attachment.
Instead of handling the attachement addition as part of the transaction, it
was done separately.  I then went back to the page where I added keyword, URL
and comment, to commit these changes.
I then got a mid-air collision warning, but it said that the other changes
were:
no changes have been made.
I thought that this was a weird way to refer to my adding of the attachment,
and as re-doing that is less work then the keyword/URL/comment, I went
ahead with the change submission.
It turned out later that the attachment was still fine, but I had overridden
the changes that Richard Guenther had made earlier.
