They do, even in wealthy NY, where million dollar bequests to the SJ Province are not infrequent. I recieve requests specifically for their infirmary in the Bronx, home to a dozen octagens. I once sent a London retiree to India, to re-visit a school he had built in the 60's. I echo your hope for the future, and will continue to anticipate ! eric.
