We already did a method to derive river centerlines based on r.thin and a method to calculate cross sections (see the scripts). It is not a method to derive the flow line of the river, but you can calculate the center line of the river. For those you need a very precise water surface

About the interaction of GRASS and HEC-RAS we have developed a grass script named r.inund.fluv, that interpolate the results of a normal 1D river model and allows to obtain a 2D flooding maps. It works quite well and you can see documentation on the add-on web page
