We to use 88305 because it is a skin, in the CPT code book there is no
option for larger codes because it is a re-excision with margins or
melanoma. If you want clarification you can go on the CAP website and get
CPT coding help. You can email them with questions and they will respond.

of the commercial insurance companies do allow anyone to bill globally
whether they do the technical or not. In those instances they can
"purchase" the "technical" work to be done somewhere else.
